Visit to Our Clinic
Please visit our clinic at your scheduled appointment time.
If you arrive more than 15 minutes late, we may ask you to reschedule your appointment.
We kindly ask you to allow enough time for your visit.
For first-time patients, we require you to fill out a medical questionnaire (online form).
To ensure a smooth process, please arrive about 20 minutes before your appointment time.

- Referral letter or medical information from your previous healthcare provider
- If you have a disability welfare handbook, disability pension, or are receiving long-term care insurance, please bring a copy of your medical certificate from your previous doctor.

Filling out the medical
questionnaire in advanceOur clinic has implemented the online medical questionnaire. If you have any concerns or questions, no matter how small, please fill it out. By completing the online medical questionnaire in advance, we can guide you directly to your consultation upon arrival. We kindly ask you to use it for a smoother process. Consultation
If you require a medical certificate, please inform us during your consultation. We will review the medical questionnaire and listen to your concerns to understand and assess your situation. Additionally, if necessary, we will conduct blood tests or psychological assessments. If hospitalization is deemed necessary, we will refer you to a psychiatric hospital with inpatient facilities, with your consent or the consent of your family.

Payment
After your consultation, once the payment is processed, we will provide you with a receipt, an external prescription, and a medical certificate if needed. Please note that consultations with only family members will be charged as private pay.
It may take approximately one hour from your arrival until the payment is completed. -

There is a coin parking lot, “Mitsui Repark Kita-Koshigaya Station West Exit,” right in front of the clinic entrance.
We will provide one service ticket per visit, so please bring your parking ticket

Kita-Koshigaya, directly to the right as you exit the West Exit of Kita-Koshigaya Station, just 0 meters away.

When you exit the ticket gate at Tobu Skytree Line’s “Kita-Koshigaya Station,” head towards the West Exit on the left side of the station.

Proceed between the building where our clinic is located and the station

Proceed about 10 meters along this path, and continue until you reach the pachinko parlor.

Please enter through the entrance with a yoga sign. Our clinic is located on the 3rd floor.
The GMC Building, where our clinic is located, is right in front of the station, but the clinic entrance is at the back of the building. It shares the same entrance as the yoga studio, making it very easy to access.
